Once you know what procedure is required, simply click the link or call the Care Concierge Team at 877-292-1541.
Care Concierge Specialists are also available to schedule your procedure and manage any required prior authorizations!
Step Two: Use the Plan
Go to the scheduled reward-eligible location for your procedure.
Step Three: Get Paid
Earn a cash reward of up to $750! Awards are issued via check and mailed to the insured within 8-10 weeks of the procedure.
